Payment
Counseling Fees are $150.00 for an hour session. Payment is due at the time of services. I can accept Visa and Master-card for the whole payment.
As a LCSW (Licensed Clinical Social Worker), I am qualified to provide you with an insurance super-bill for your Mental Health Insurance. Clients cover their costs, $150 a session, and I will provide the paperwork for your insurance company at the first each month.

Forms
You can download the intake form(s) that meet your needs. If possible, please mail back the forms before the first appointment. If time does not allow mailing, make sure you bring the forms to the first appointment to avoid filling them out again at the office. If you have questions please email or call.
