Payment
Counseling Fees are $150.00 for an hour session. Payment is due at the time of services. I can accept Visa and Mastercard for the whole payment or your co-pay.
As a Licensed Clinical Social Worker, I am qualified to bill your Medical Health Insurance. Clients cover their deductible and co-pays. I currently bill the following health insurance programs. If yours is not on the list, contact me and I’ll investigate if you are eligible for reimbursement.

I provide an insurance super-bill that you can submit to your insurance company.
Forms
You can download the intake form(s) that meet your needs. If possible, please mail back the forms before the first appointment. If time does not allow mailing, make sure you bring the forms to the first appointment to avoid filling them out again at the office. If you have questions please email or call.
